Twenty years after stealing £16,000 in drug money from his friends and making a new life in Amsterdam, 46-year old Mark Renton, who no longer uses heroin, suffers a heart attack in a gym. Francis "Franco" Begbie is in prison and attacks his lawyer after his sentence is extended for another five years. Daniel "Spud" Murphy has returned to a cycle of heroin addiction after separating from his partner, Gail, and losing custody of his teenage son, Fergus. Simon "Sick Boy" Williamson runs a failing pub called Port Sunshine, abuses cocaine, and engages in blackmail schemes with his Bulgarian girlfriend, Veronika.
